The score sums 6 criteria: Asthma; Eosinophilia > 10 % on differential white cell count; Mononeuropathy or polyneuropathy; Migratory or transient pulmonary infiltrates on imaging; Paranasal sinus abnormality; Biopsy containing a blood vessel with extravascular eosinophils. Totals are then banded: any score - Criteria not met: fewer than 4 criteria - does not classify as EGPA; 4 criteria or more - Criteria MET: ≥4 of 6 criteria - classifies as EGPA (sensitivity 85%, specificity 99.7%). Assess for cardiac involvement, which drives mortality.
